Clinical synopsis
A 43-year-old woman presented to the dermatology clinic with a pigmented papule on the flexor surface of her right forearm of eight months duration (Fig. 1). It started initially as a flat brown discoloration of the skin that gradually increased in size in the first five months. The lesion then became raised over the following three months. There were no symptoms or bleeding. There was no history of excessive exposure to sunlight and no family history of cutaneous malignancy. On examination, there was a 4mm, symmetrical, slightly raised brown-red papule with no surrounding erythema or depigmentation. A full cutaneous examination revealed a few lentigines and nevi; there was no lymph node enlargement. A complete excision was performed.
